Change of address

If you change your address, you must report your new address in writing no later than five days after having moved, and you must choose a family doctor/GP in the municipality to which you are moving.

You can report a change of address by

- using: Change of address - self-service solution (Page in danish)

- or by visiting the nearest Citizens’ Services centre.

Changing your GP in connection with a change of address is free. When moving to a new municipality, you must choose a new GP/family doctor within a distance of 15 km from your new address. You can choose your GP via the self-service solution. If you want to keep your current GP, you can apply to do so on a special form for using a GP located more than 15 km from your new address – your GP must complete the bottom part of the form.
